The tongue of the trailer jumped off the hitch and pinched the employee's right index finger, causing a fracture, a torn ligament, the loss of the nail, and the amputation of about 3/8 inch of the tip.

After delivering a load of stone to the jobsite, the driver of the dump truck was checking to see why the tailgate of the dump truck was not closing properly. When he pushed the tailgate, it returned, trapping the fingers on his left hand between the bed and the gate. The ring and middle fingers were amputated to the first joint.

On June 14, 2019, an employee was dumping approximately 3 tons of asphalt that was left inside the bed of a dump truck from the night before. The employee raised the dump truck and then exited the truck. While the employee was standing at the rear of the truck, the asphalt broke loose and fell onto the employee, pinning the employee between the pavement and the tailgate of the dump truck. The employee suffered burns, lacerations, broken bones, and the amputation of toes.

Tri-axle dump trucks were importing stone to the worksite. After one of the trucks emptied its load the injured employee noticed stone on the tail of the body where the tailgate would close. The employee notified the driver who, again, raised and lowered the bed of the tri-axle to dislodge the debris. When the bed lowered the employee's left hand was caught in a pinch point between the truck body and the tailgate resulting in amputation of the middle and ring fingertips.

While removing the seat from the cab, the employee lost his footing and fell 6.5 feet to the ground. The employee suffered compression fractures to multiple vertebra and was hospitalized.

The employee sustained a knee fracture, a shoulder injury and a forehead abrasion.

An employee was using a handheld shovel to level out a ditch for a retention pond pipe. At the same time, an off-road truck was being loaded beside the retention pond. A rock (2 to 3 inches in diameter) then fell off the dump truck and rolled downhill into the ditch, striking the employee. The employee suffered back and left side injuries.

An employee was helping to load a dump truck with construction debris when, unbeknownst to him, another employee lowered the truck's lift gate. When he attempted to walk backward out of the truck, the lift gate was no longer there to support him and he fell 3.5 feet to the lift gate below, fracturing his L1, L2, and L3 vertebrae.

An employee was climbing the steps into his dump truck. When he reached the top step, which was frozen and slick, he fell approximately 4 feet to the ground and landed on his left side on a frozen rut. He lost consciousness and sustained several broken ribs, a broken T-10 disc, and a punctured left lung. He was hospitalized.

An employee was underneath a disabled tri-axle dump truck attempting to release a driveshaft from the bound-up transmission. When the driveshaft was disconnected and came off, the truck drifted and dragged the employee who was making the repairs. The employee was hospitalized and suffered a broken collarbone, four broken ribs, and a bruised kidney.

On August 18, 2017, an employee was leveling a yard and dumping rock from a dump truck bed. While attempting to close the tailgate, the employee discovered a rock was lodged in the tailgate. The employee lifted the tailgate, which released the rock. The tailgate then came down and pinned the employee's right index finger against the dump truck bed. The employee was hospitalized and the finger was amputated to the first knuckle.

The injured employee was on the passenger side while the coworker was on the driver side. The coworker entered the cab and started the engine. The injured employee contacted the radiator's belt and pulley at that moment, which amputated the end joint of the employee's right index finger.
